Record-Level Data Request

If you are interested in obtaining de-identified record-level data for your own analyses, please see the instructions below for each available data source.

 

Vital Statistics and Hawaii Health Survey (HHS)

To acquire record-level data on births and deaths from Hawaii’s Vital Statistics or survey data from HHS, please send an email with your detailed request to profiles@hhdw.org. We will connect you with the appropriate people.

Alternatively, Hawaii-level birth and death data can be obtained from the National Vital Statistics data files located here.

 

Behavioral Risk Factor Surveillance System (BRFSS)

The Centers for Disease Control and Prevention (CDC) offer record-level BRFSS data for download here.  More  details on the the national data sets can be found here.  While this is the easiest way to get record-level BRFSS data, the data set from CDC does not include state-added questions and uses a more broad race/ethnicity categorization.  If you need data from state-added questions or if you want the more detailed race/ethnicity classification, you should submit a proposal to the Hawaii State Department of Health (DOH) for record-level BRFSS data. Hawaii-specific data can be merged with the national data set using the annual sequence number.

In order to prepare your data request, please be sure to review the DOH Data Sharing Policies and Procedures and use the BRFSS Data Request Codebook to specify the variable names for the standard  core and optional questions as well as the state-added questions.  Note: Since the BRFSS is not designed to generate population estimates at the zip code-level and to protect the privacy of the participants, zip code level data will not be released. Aggregations of zip codes by community or school complex are available and custom groupings may also be provided upon request.

Youth Risk Behavior Survey (YRBS)

The Hawaii YRBS data is not available on the Centers for Disease Control and Prevention (CDC) website at this time. In order to acquire record-level YRBS data, you can submit a proposal to Hawaii State Department of Health (DOH). Please be sure to review the DOH Data Sharing Policies and Procedures.

Record-level YTS data is not available online.  You can submit a proposal to the Hawaii State Department of Health (DOH) for record-level YTS data.  Please be sure to review the DOH Data Sharing Policies and Procedures.

Pregnancy Risk Assessment Monitoring System (PRAMS)

Record-level PRAMS data can be acquired through the CDC here.   You can also submit a proposal to the Hawaii State Department of Health (DOH).  To submit a Hawaii PRAMS data request

  1. Review and acknowledge Collaborative Studies Using Hawaii PRAMS Data guidance.
  2. Complete a research proposal using the suggest format in the Hawaii PRAMS Full Proposal Approval Form.
  3. Review and sign the PRAMS Data Sharing Agreement.
  4. Review and sign the Additional Guidelines for Researchers form.

Please type or print clearly, and sign or acknowledge all the forms. Failure to provide the information described in 1-4 may result in a delay.  Please email all four documents to Jane Awakuni  (jane.awakuni@doh.hawaii.gov). You may also submit them by mail or fax to:
