The Hawai‘i Diabetes Plan 2030 by the Hawai‘i State Department of Health reflects a common vision for preventing and managing diabetes.
The Diabetes Plan 2030 Tracker is designed to monitor the burden of diabetes and keep all stakeholders updated on the state’s progress. It compiles 48 indicators from national and state surveillance systems like the Behavioral Risk Factor Surveillance System (BRFSS) and the Youth Risk Behavior Survey (YRBS). The tracker is organized into 7 categories:

- The percentage of adults who have ever been diagnosed with diabetes has increased over time, from 8.4% in 2011 to 11.5% in 2024. The Diabetes Plan Target is 10.5%.
- The age-adjusted death rate due to diabetes as an underlying or contributing cause of death is 62.8 deaths per 100,000 population. This value is lower than the US value (79.4), but is increasing significantly over time.
- 62.9% of adults with diabetes or prediabetes have worked with a health care provider to create a self-management plan.
- 10.4% of adults with diabetes could not afford to eat a balanced meal in the past 30 days. This value increases to 21.4% among individuals between 0-130% of the federal poverty level.
- Almost 1 in 10 (9.1%) of adults with diabetes lacked reliable transportation that kept them from medical appointments, meetings, work, or from getting things needed for daily life in the past 12 months.
