YRBS – Record Level Data Request
The Centers for Disease Control and Prevention (CDC) has created YRBS combined datasets for high school surveys conducted from 1991-2019 and middle school surveys conducted from 1995-2019. These datasets contain data from the National YRBS (high school only) as well as state and large urban school districts. Click here to find out more about these datasets and to download data.
While this is the easiest way to get record-level YRBS data, the CDC data does not include state-added questions, including Hawai‘i-specific race/ethnicity.
If you want state-added questions, more detailed Hawai‘i-specific race/ethnicity categories or the HHDW conformed indicators, you should submit a proposal to the Hawai‘i State Department of Health (DOH) and the Department of Education (DOE) for record-level YRBS data. This helpful video explains the data request process.
In order to prepare your data request, please be sure to review the DOH Data Sharing Policies and Procedures and use the Hawai‘i YRBS Data Request Codebook to identify each variable by survey variable name and year(s). Copies of the survey instruments can be found here.
Complete the following three documents and send them to profiles@hhdw.org. Please type or print clearly. Complete and sign all forms. Failure to provide the required information may result in a delay.
