YTS – Record Level Data Request
Record-level YTS data is not available online. You can submit a proposal to the Hawaiʻi State Department of Health (DOH) and the Department of Education (DOE) for record-level YTS data.
In order to prepare your data request, please be sure to review the DOH Data Sharing Policies and Procedures and use the Hawai‘i YTS Data Request Codebook to identify each variable by survey variable name and year(s). Copies of the survey instruments can be found here.
Complete the following three documents and send them to profiles@hhdw.org. Please type or print clearly. Complete and sign all forms. Failure to provide the required information may result in a delay.

(Note: Use the Hawai‘i YTS Data Request Codebook to complete the proposal.) - DOH Data Use Agreement
(Note: Anyone with access to the data must include their signature on this agreement.) - DOH Data Sharing Agreement
